Here is the procedure to download this package:

1. If you have Adobe Acrobat Reader properly installed on your computer, the package will download in a separate window below. If you do not have this program, click this text to download and install it (it is free!!) before proceeding.
2. Use the controls on this small window to Save the form on your computer (remember where you save it!) by clicking on the save icon (looks like a small floppy disk) using the button on the upper left immediately above the form, follow the arrow to the left (in this case, do not use the save command from the file menu on your computer).
3. You can exit the Web Browser now (or you can leave it open, if you like). Then open the Adobe Acrobat Reader program on your computer, and then open the “2010 Participant Package.pdf” file that you just downloaded. Use the “open” command from the “file” menu of the Adobe Acrobat Reader program to open this file.
4. Once open, use the “print” command from the “file” menu to print the form.
5. Fill out the form, mail it to the address listed on the form or fax it to (703) 995-0385. (If you want to send it as an e-mail attachment, I suggest you download the form only via, click this text to go to that form.)
